I 1. Comply with Title VI of the Civil Rights Act of 1963, and in accordance with Title VI of the Act, <br />no person in the United States shall, on the grounds of race, color, sex, or national origin be <br />excluded to discrimination under a program activity for which the applicant receives federal <br />financial assistance. <br />12. Maintain a hazard -free and sanitary working environment for participants, and make certain all <br />safety rules are followed. <br />13. Maintain participant time and attendance records on a daily basis and ensure the participant and <br />supervisor sign time cards. <br />14. Maintain a copy of the worksite agreement and Work Experience handbook on hand at worksite, <br />and medical release (if required). Post Worker's Compensation Information Bulletin in an area <br />visible to participants and supervisors. <br />15. Complete participant time cards on a daily basis. Time card should be reviewed on the last day of <br />the pay period and ready for pick up on designated day. <br />16. Participants shall be allowed release time for counseling, skill/remedial education, labor market <br />orientation, and/or other activities when requested by the grantee. <br />17. Work Experience staff will be notified of participant employment problems at the earliest possible <br />time by the worksite supervisor. <br />18. Report all accidents and injuries no later than 24 hours after the accident occurs. <br />19. Agency worksite supervisors will allow designated Work Experience staff, State, and Federal <br />officials, to perform on-site monitoring and visitation while participants are present. <br />20. Participants will not be released from work without pay, unless first cleared with Work <br />Experience staff. <br />21. Follow rules and regulations governing the Work Experience program. <br />B. The City of Santa Ana Work Experience Program ensures and agrees to fulfill the following: <br />1. Provide an orientation to agency worksite supervisors concerning Work Experience guidelines, <br />California Labor Laws, California Child Labor Laws and departmental procedures. <br />2. Provide handbooks to participant and supervisors that provides an orientation to the Work <br />Experience program and their responsibilities as a participant/supervisor. <br />3. Provide participant time cards to worksites. <br />4. Process Work Experience participant payroll and pay participant wages, <br />5. Monitor program and worksite activities, and provide assistance to participants and supervisors <br />when needed, <br />6. Provide total medical costs associated with injuries that may occur on the job through the Workers' <br />Compensation Program. <br />Rev. 08/12 <br />
