2014/15 1 2010/11- 2014/15 CITY OF SANTA ANA CONSOLIDATED PLAN <br />ACTION PLAN <br />• Consultation with Continuum of Care (CoC) —The City must consult with the CoC that <br />serves the regional homeless <br />• The HEARTH Act also required changes to the way ESG funds are allocated; specifically, <br />only 60 percent of the City's ESG grant may be used for shelter and outreach services <br />The Act also modified the definition of homelessness and allows ESG grantees to use some <br />Continuum of Care funding for people who are not homeless under HUD's definition, but <br />are homeless under definitions of homelessness used by other federal agencies. <br />Additionally, the HEARTH Act added a definition for a person that is at imminent risk of <br />homelessness or where a family or unaccompanied youth is living unstably. The Hearth Act <br />also required that the City make certain modifications to its Consolidated Plan, Action Plan <br />and year -end performance reports. The 2014 -2015 Action Plan takes into account required <br />modifications. Taking the HEARTH Act into account, during the 2014 -2015 Program Year, <br />the City will undertake the following homeless housing /service activities: <br />HOMELESS ASSISTANCE - OUTREACH & SUPPORTIVE SERVICES <br />Santa Ana's homeless strategy calls for the provision of outreach and support services to <br />assist individuals and families that are homeless or at risk of becoming homeless. ESG <br />resources will be provided to local nonprofit organizations and the City's police department; <br />outreach services will be provided to an estimated 793 individuals during FY 2014 -2015. <br />Additionally, the City has allocated CDBG funds to fund the HEART Program, which will be <br />used to supplement ESG funding for outreach /assessment. Planned accomplishments for <br />the 2014 -2015 Program Year are outlined in the LISTING OF PROPOSED PROJECTS— EXHIBIT3. <br />HOMELESSNESS PREVENTION <br />A key component of the City's homeless strategy is to prevent individuals and families from <br />slipping into homelessness. This is accomplished by providing monetary assistance and <br />counseling /support services to at risk households. The City's continuum of care strategy <br />calls for providing ESG resources to agencies that provide this type of assistance. An <br />estimated 108 individuals will be assisted with homelessness prevention services during <br />2014 -2015. Planned accomplishments for the 2014 -2015 Program Year are outlined in the <br />LISTING OF PROPOSED PROJECTS- EXHIBIT 3. <br />SHELTER <br />The City will provide funds to shelter providers via the ESG program. These funds will help <br />offset the costs of providing emergency shelter, transitional housing and support services <br />(e.g., legal assistance) for Santa Ana's homeless.
