<br />License or Identification Card; <br />(9) If the Medical Marijuana Collective is a corporation, a certified <br />copy of the Collective’s Secretary of State Articles of Incorporation, Certificate(s) <br />of Amendment, Statement(s) of Information and a copy of the Collective’s <br />Bylaws; <br />(10) If the Medical Marijuana Collective is an unincorporated association, <br />a copy of the Articles of Association; <br />(11) The name and address of the applicant’s current agent for Service of <br />Process; <br />(12) A copy of the Medical Marijuana Collective Operating Standards, <br />listed in Section 18-613, containing a statement dated and signed by the <br />responsible party on-site stating under penalty of perjury, that they read, <br />understand and shall ensure compliance with the aforementioned operating <br />standards. <br />18-617.3 Application timing and lottery selection. <br />(a) On November 18, 2014, at 8:00 a.m., the Planning and Building Agency <br />will make applications for a permit available at the planning counter in City Hall and <br />online on the City’s website. On December 1, 2014, at 8:00 a.m., the Planning and <br />Building Agency will begin collecting applications at the planning counter or other <br />location at City Hall designated at least twenty-four (24) hours in advance by the <br />Executive Director of the Planning and Building Agency. Applications shall be <br />submitted in person during business hours until 5:00 p.m. on December 8, 2014.Multiple <br />submissions from the same applicant at the same location will result in immediate <br />disqualification from the registration process. Applications received within the time <br />period noted above will be placed into a lottery system which will be randomly selected <br />by the Executive Director or his/her designee. <br />(b) The lottery will be conducted in City Council Chambers or a City <br />conference room accessible to the public on December 22, 2014 at 3:00 p.m. <br />Applications will be selected through the lottery system at random by the Executive <br />Director of the Planning and Building Agency or his/her designee. <br />(c) After the lottery, if during review any application is determined to be <br />incomplete, the applicant shall be notified in writing within ten business days of the date <br />the application is determined to be incomplete. <br />(d) The Collective shall have 30-days from the date of notice set forth in <br />subsection (c) to complete the application.
