Submittal <br />Requirements <br />Submittal Format and Content <br />The response to this RFQ should be designed in such a way as to best represent the consultant's <br />qualifications and experience based on the information provided in the Qualifications section. <br />At a minimum the response should include: <br />• Cover page <br />• Signed letter of transmittal <br />• Table of contents <br />• Demonstration of qualifications — this may include sample documents or any other examples of <br />creative approaches to community engagement which the consultant has developed and <br />implemented. There is no restriction on the size, number or form of presentation media used for <br />these examples. Samples demonstrating experience in other languages are welcome. <br />• Conceptual outline of the process to develop the plan including a project timeline <br />• Resumes for staff who would be involved in the project <br />• Rate sheet <br />Consultant teams selected to move on to the interview phase will also be given the opportunity to <br />make a presentation to the selection panel. <br />Submittal Date <br />All responses must be received no later than 5 pm on Thursday, October 16, 2014. <br />Submittal Quantity and Contact <br />Please provide 10 copies of the submittal in either printed or digital format to: <br />Karen Haluza.
