c. Proposed revenue sharing fee structure. <br />ADDITIONAL INFORMATION <br />City Business License <br />The selected proposer shall be required to obtain a City of Santa Ana Business license <br />within thirty (30) days of selection and must provide a copy to the City projects manager or <br />designee prior to commencing any work in Santa Ana. A current business license must be <br />maintained for the entire term of the contract. Failure to comply with this requirement will <br />result in immediate cancellation of the City's contract. Procedure to obtain a City of Santa <br />Ana business license is available on the City's website: http://www.santa- <br />ana.orq /finance /business /business license.asp <br />Insurance <br />The Operator shall, at its own cost, maintain insurance acceptable to City in full force and <br />effect throughout the term of this Agreement. The policy of policies of insurance <br />maintained by Operator shall provide the following limits and coverage: <br />1. Commercial General Liability (CGL) or Garage Liability: Insurance Services <br />Office (ISO) Form CG 00 01 12 07 covering CGL on an "occurrence" basis, <br />including products - completed operations, personal & advertising injury, with limits <br />no less than $2,000,000 per occurrence. If a general aggregate limit applies, either <br />the general aggregate limit shall apply separately to this project/location or the <br />general aggregate limit shall be twice the required occurrence limit. <br />2. Garage Keeper's Legal Liability: with limits no less than $2,000,000 covering the <br />Operator and the City against claims, liabilities, losses, or suits covering exposures <br />for loss of or damage to vehicles in Operator's care, custody and control. This <br />coverage shall be subject to a deductible provision of not more than $1,000 for <br />each loss. <br />3. Commercial Crime including Employee Dishonesty: with limits not less than <br />$100,000 for Employee Dishonesty and $10,000 for depositors forgery, loss of <br />money and securities, inside or outside the premises and including safe robbery or <br />burglary <br />4. Workers' Compensation: as required by the State of California, with Statutory <br />Limits, and Employer's Liability Insurance with limit of no less than $1,000,000 per <br />accident for bodily injury or disease. <br />a. "This insurance shall not be cancelled, limited in scope of coverage, or non - <br />renewed until after thirty (30) days' written notice has been given to the <br />Community Development Agency of the City of Santa Ana." <br />25B -20 <br />
