The page limitation includes all appendices, <br />attachments, and supplemental information. <br />The following information is required: <br />Cover Letter — A letter signed by a principal or authorized representative who can <br />make legally binding commitments for the entity. <br />Firm and Personnel Experience:A profile of the firm's experience, including the <br />names and experience of personnel and subconsultants who will be providing <br />services. At a minimum, this should include the project manager /principal agent, <br />associates in charge when project manager /principal agent is unavailable, key <br />personnel, and an organization chart identifying only those who will perform work <br />for the proposed project and the percentage of each individual's time devoted to <br />this project. The project manager /principal agent shall be the primary contact <br />person to represent your firm and will be the person to conduct the presentation, <br />if invited for an interview. <br />The firm shall identify at least two (2) ISA Certified Arborists who have at least 5 <br />years' experience in the field of urban forest management that will be responsible <br />for providing project management throughout the life of the contracts as well as a <br />full -time English speaking Project Supervisor who is an ISA Certified Arborist, <br />capable of communicating with any City representative and be authorized to act <br />on behalf of the firm. <br />3. List of staff qualification including but not limited to: <br />a. Certified Arborists employed by the firm. <br />b. Certified Tree Workers employed by the firm. <br />C. Certified Utility Arborists employed by the firm. <br />d. Utility Line Clearance Specialist employed by the firm. <br />e. Certified Wildlife Protectors employed by the firm. <br />f. California State Licensed Pest Control Advisor employed by the firm. <br />g. California State Licensed Pest Control Applicator employed by the firm. <br />h. Consulting Arborist employed by the firm to consult on tree health issues. <br />i. Technicians providing technical support for inventory software. <br />4. Documentation of the technical ability and experience similar in scope to the <br />project. <br />a. A description of previous experience, including urban forestry <br />management projects of similar nature and scope. <br />b. A written description of the proposed software program to be used to <br />manage the City's Tree Inventory and firm's ability to provide accurate <br />inventory updates for all trees serviced. <br />C. A statement describing the firm's ability to provide tree inventory data for <br />the City's Geographic Information System (GIS). Data shall be <br />compatible with the City's GIS program. <br />d. A written description of work activities that includes but not limited to the <br />following activities: <br />i. Grid pruning <br />ii. Service responses <br />iii. Planting <br />V. Removals <br />V. Emergency response protocol <br />e.
