APPROPRIATION ADJUSTMENT AND AGREEMENT TO IMPLEMENT <br />THE ORANGE COUNTY PARTNERSHIP TO IMPROVE COMMUNITY <br />HEALTH GRANT PROJECT {STRATEGIC PLAN 5, 4131 - Public Works <br />Agency <br />MOTION: <br />AGMT NO. 2015-086 - Authorize the City Manager, and the Clerk of <br />the Council to execute a sub -grant agreement with Community <br />Action Partnership of Orange County, subject to non -substantive <br />changes by the City Manager and City Attorney, to implement the <br />Orange County Partnership to Improve Community Health project, <br />for the one-year period ending September 29, 2015, in an amount <br />of $145,000 with provisions for two additional one-year options <br />contingent on adequate funding from the Centers for Disease <br />Control exercisable by the City Manager and the City Attorney, <br />utilizing project grant funding as awarded. <br />2. Approve an appropriation adjustment. (Requires five affirmative <br />votes) <br />APPROPRIATION ADJUSTMENT NO. 2015-180 - Recognizing <br />$145,000 in Partnership to Improve Community Health grant funds into <br />the Emergency and Health Fund revenue account and appropriating <br />the same in the expenditure account. <br />SPECIFICATIONS — PURCHASE OF EQUIPMENT AND SERVICES <br />22A.
