Evaluation of Existing Bridge Hinge - The Consultant shall provide an evaluation of the condition of the <br />existing bridge by means and methods that provide for a clear understanding of the problems and issues <br />associated with the potential damage. The Consultant shall provide details that reflect this condition on <br />plan sheets that give the City a clear understanding of the issues. <br />Proposed Repair Work (If Any) of the Existing Bridge - The Consultant shall provide recommendations for <br />the repair of the existing bridge by means and methods that provide for a clear understanding of the <br />proposed work to be done to bring the bridge up to current and acceptable standards. <br />Preliminary Engineer's Estimate for Repair Work - If repair work is necessary, the Consultant shall provide <br />detail cost estimate that reflect this proposed work, the Consultant shall prepare a quantity takeoff of repair <br />work to be done to restore the bridge to current and acceptable standards. This includes, but is not limited <br />to, bridge hinge repair, potential shoring of bridge during construction, traffic control, hazardous waste <br />containment and disposal, and restriping. <br />Summary Report of Findings and Recommendations - The Consultant shall prepare a report <br />summarizing all the task items listed above in a report that clearly outlines and identifies the required work <br />presented in this proposal. The consultant shall also submit electronic spreadsheet that will included as a <br />minimum for each bridge the priority for funding(lowest number is highest priority), county geographic <br />location, name of implementing agency, bridge number, facility carried, feature intersected, location, <br />sufficiency rating, SD/FO status, all Preventive Maintenance (PM) treatment for each bridge, unit cost of <br />PM (total deck area basis), federal funds, running summary of federal funds, local funds, and running <br />summary of local funds (See sample spreadsheet in Appendix 4). The Consultant shall include any and all <br />attachments that allows for this report to be a "stand alone" document. <br />Address City Comments and Provide Final Report - The City will review the draft report and provide <br />comments, where appropriate, with recommendations, if required. The Consultant shall address City <br />comments and prepare a final report to the City. The Consultant shall provide the City with the following <br />final deliverables: <br />• Two (2) hard copies of the final report (by September 21, 2015) <br />• One (1) electronic copy ; all documents shall be submitted in MS Word <br />and PDF, exhibits shall be submitted in Microstation format. <br />Additional program requirements and constraints are available at the following website: <br />htW://www.dot.ca.govihq/LocalProgmms/ <br />City of Santa Ana RFP 15 -047 <br />2015 <br />
