Agreement is considered as a default and at Subgrantor's option, subgrantor <br />can refuse future renewal of this Agreement. <br />This Grant has been assigned the Federal Award Identification Number <br />(FAIN) U58DP005861. Subgrantee must document the assigned FAIN on all <br />communication and correspondence. <br />2. Term and Amount of Agreement <br />a. Term of this Agreement is from October 1, 2014 to Sept 29, 2015; <br />b. There are two (2) 1 -year options which are contingent on adequate <br />funding from the Centers for Disease Control and Prevention (CDC) <br />and satisfactory programmatic progress of Subgrantee. Subgrantee <br />must give ninety (90) day notice in writing as prescribed herein to <br />Subgrantor.to exercise each option; <br />c. The maximum amount of funding from October 1, 2014 to Sept 29, <br />2015 is $145,000; and <br />d. This Agreement shall terminate if the agreement between Subgrantor <br />and CDC is terminated and upon such termination, Subgrantor shall <br />have no further liability to Subgrantee. <br />3. Program Requirements, Standards and Guidance <br />a. This award is based on the application submitted to Subgrantor and as <br />approved by CDC and is subject to the terms and conditions <br />incorporated either directly or by reference in the following: <br />i. The Grant program legislation and program regulation, as <br />amended from time to time, cited in the Notice of Award (NOA) <br />between CDC and Subgrantor; (a copy of which is attached <br />hereto and incorporated herein as Exhibit "B "); <br />ii. The restrictions on the expenditure of federal funds in <br />appropriation acts to the extent those restrictions are pertinent <br />to this award; <br />iii. The HS Grants Policy Statement, including addenda in effect as <br />of the beginning date of the budget period; and <br />iv. The terms and conditions cited in this Agreement. <br />b.
