The <br />SOAR Program is beneficial in providing <br />assistance to clients who are completing <br />SSI applications as well as applications <br />for other mainstream benefits. <br />Offered at entry, <br />or as requested <br />by client <br />Mercy House <br />Housing <br />Stabilization Case <br />Manager <br />Assess homeless <br />individuals to <br />prescribe <br />treatment/services <br />Mercy House Housing Stabilization Case <br />Managers will assess clients at entry and <br />as needed for referrals to services <br />including medical and mental health <br />care. Medical and mental health <br />assessments will be completed through <br />community resources and services. <br />At entry and as <br />needed <br />Mercy House <br />Housing <br />Stabilization Case <br />Manager <br />Linkages to Medical <br />and Mental Health <br />Treatment <br />Medical and mental health treatment will <br />be referred out to other agencies or <br />medical professionals.
