PROJECT DESCRIPTION <br /> <br /> <br />11 | Page <br /> <br /> <br />SUPPORTIVE SERVICES FOR TENANTS <br />Supportive services are designed with a focus on intensive case management, with emphasis on behavioral <br />health and primary health issues, independent living skills, and employment/educational goals. Intensive case <br />management includes individualized service planning and goal setting, coordination of services, counseling and <br />support, crisis intervention, intensive assistance with accessing mainstream services and other community- <br />based resources. <br />These Services will be designed to meet the specific needs of the residents and include: <br />Services <br />Service Description Frequency Service Provider <br />Outreach and Intake Chronically homeless individuals will be <br />assessed by community outreach teams <br />and referred to the housing program <br />through the local Coordinated Entry <br />System. Mercy House Housing <br />Stabilization Case Managers will provide <br />entry and intake support to referred <br />clientele. <br />At entry <br /> <br />Community <br />Outreach Teams, <br />Coordinated Entry <br />Team, Mercy House <br />Housing <br />Stabilization Case <br />Manager <br />Housing Placement <br />Assistance <br />Applicants will be partnered with a <br />Housing Stabilization Case Manager to <br />assist them with obtaining necessary <br />housing documents and move-in <br />support. The Housing Stabilization Case <br />Manager will also assist in any appeals or <br />reasonable accommodations requests as <br />desired by the applicant. <br />At entry <br /> <br />Mercy House <br />Housing <br />Stabilization Case <br />Manager and <br />Leasing Agent <br />Assessment of Needs A Housing Stabilization Case Manager <br />will be assigned at entry to identify the <br />individual’s needs and barriers. A <br />Housing Stabilization Plan will be created <br />for the client to guide supportive <br />services. <br />Offered at entry <br />and during <br />monthly case <br />management <br />sessions <br />Mercy House <br />Housing <br />Stabilization Case <br />Manager <br />Case Management Aimed at identifying and addressing <br />barriers to maintaining permanent <br />housing.
