The <br />SOAR Program is beneficial in providing <br />assistance to clients who are completing <br />SSI applications as well as applications <br />for other mainstream benefits. <br />Offered at entry, <br />or as requested <br />by client <br />Mercy House <br />Housing <br />Stabilization Case <br />Manager <br />Assess homeless <br />individuals to <br />prescribe <br />treatment/services <br />Mercy House Housing Stabilization Case <br />Managers will assess clients at entry and <br />as needed for referrals to services <br />including medical and mental health <br />care. Medical and mental health <br />assessments will be completed through <br />community resources and services. <br />At entry and as <br />needed <br />Mercy House <br />Housing <br />Stabilization Case <br />Manager <br />Linkages to Medical <br />and Mental Health <br />Treatment <br />Medical and mental health treatment will <br />be referred out to other agencies or <br />medical professionals. Housing <br />Stabilization Case Manager will work to <br />ensure that client’s medical needs are <br />being met and advocate on their behalf. <br />At entry and as <br />needed <br />Orange County <br />Health Care <br />Agency; Share Our <br />Selves (SOS) <br />Assistance in <br />obtaining other <br />resources and <br />supports <br />Housing Stabilization Case Managers will <br />identify the client’s needs regarding <br />furniture, household items, <br />transportation, job training and <br />placement and connect them to the <br />adequate agency or resource which may <br />assist them in meeting these needs. <br />At entry, as <br />needed <br />Mercy House <br />Housing <br />Stabilization Case <br />Manager <br />Veterans benefits Assessment for Veteran-specific <br />services <br />At entry and as <br />needed <br />VA Staff/Case <br />Managers <br />Veteran mental and <br />physical health care <br />Connection to Veteran-specific mental <br />and medical health care services <br />including psychological counseling. <br />At entry and as <br />needed <br />VA staff/ Case <br />Managers <br />3-22
