Applicants will be given a reasonable amount of time to obtain <br />required documentation and verifications. If requested, Mercy House will assist with the <br />porting process if needed by the tenant-based voucher. Mercy House will also assist in any <br />appeals or reasonable accommodations requests as desired by the applicant. <br /> <br />The following is a summary of resident selection procedures to be used at the property. <br />These are subject to the SAHA’s review and approval: <br /> <br />a) All qualified applicants that are not able to move in will be placed on a waiting list and <br />shall receive a letter informing them of their status. <br /> <br />b) Those applicants not selected for occupancy shall be provided with written notification <br />stating the reasons for their ineligibility. Such reasons might include but are not limited to <br />the following: over income, insufficient income (inability to pay the rent), and criminal <br />history. Housing First practices will guide selection criteria to avoid automatic applicant <br />denials. Those applicants rejected for residency shall be provided with an opportunity to <br />appeal. The appeal process shall allow for the applicant to provide supplemental <br />information to the application including additional third party verification and/or testimony. <br /> <br />c) Eligible applicants will be provided with third-party verification forms to be filled out <br />by the income provider, or themselves if they are self-employed. Self-employed persons <br />must provide a signed affidavit, a year to date Profit and Loss Statement and two years <br />Federal Tax Returns, including all schedules submitted. Third-party verification forms may <br />be waived for those chronically homeless applicants. <br /> <br />d) Upon third-party verification of income, the applicant shall either be offered residency <br />or their name shall be added to the waiting list and time-dated. The waiting list will be <br />updated on a regular basis (every six months). Applicants on the waiting list will be <br />required to contact the manager (in writing) at least every 6 months in order to remain <br />on the waiting list. All revisions to applicant’s data on the waiting list will be maintained by <br />Solari’s Property Manager, including date of change and person noting change. The <br />waiting list will be kept electronically using Yardi Systems Software to establish a clear <br />audit trail. The waiting list will be made available for review by the Housing Authority, <br />upon written request to the Owner. <br /> <br />3-51
