<br /> <br /> Page 4 of 5 form HUD-50075-HP (12/2014) <br /> <br /> <br /> <br /> <br />B.4. <br /> <br />Most Recent Fiscal Year Audit. <br /> <br />(a) Were there any findings in the most recent FY Audit? <br /> <br />Y N <br /> <br /> <br />(b) If yes, please describe: <br />There were two findings. <br /> <br />The first finding stated, “During our review, we noted that the City did not have written procedures to implement the requirements of 2 CFR <br />section 200.305 Payment during fiscal year 2015.” The Cause was, “the City was unaware that this grant was subject to the requirements of <br />the Uniform Guidance, thus did not implement changes during fiscal year 2015”. The Effect was, “Failure to have a written pr ocedure to <br />ensure the compliance with 2 CFR section 200.305 Payment may result in noncompliance with Uniform Guidance requirements.” <br /> <br />The second finding stated, “During our review of compliance with the uniform guidance requirements, we noted that the City did not have <br />written procedures for determining the allowability of costs and the terms and conditions of the Federal award in fiscal year 2015. However, <br />the City developed and implemented the written procedures during fiscal year 2016.” The Cause was, “the City was unaware tha t this grant <br />was subject to the requirements of the Uniform Guidance, thus did not implement changes during fiscal year 2015.” The Effective was, <br />“Failure to have a written policy for determining allowability of costs and the terms and conditions of the Federal award may result in <br />noncompliance with Uniform Guidance requirements.” <br /> <br /> Other Document and/or Certification Requirements. <br /> <br />C.1 <br /> <br /> <br /> <br /> <br /> <br /> <br />Certification Listing Policies and Programs that the PHA has Revised since Submission of its Last Annual Plan <br /> <br />Form 50077-ST-HCV-HP, Certification of Compliance with PHA Plans and Related Regulations, must be submitted by the PHA as an electronic <br />attachment to the PHA Plan. <br /> <br />C.2 <br /> <br />Civil Rights Certification. <br /> <br />Form 50077-ST-HCV-HP, Certification of Compliance with PHA Plans and Related Regulations, must be submitted by the PHA as an electronic <br />attachment to the PHA Plan. <br /> <br /> <br />C.3 <br /> <br />Resident Advisory Board (RAB) Comments. <br /> <br />(a) Did the RAB(s) provide comments to the PHA Plan? <br /> <br />Y N <br /> <br /> <br />If yes, comments must be submitted by the PHA as an attachment to the PHA Plan. PHAs must also include a narrative describin g their analysis of <br />the RAB recommendations and the decisions made on these recommendations. <br /> <br />The City analyzed the recommendations of the Resident Advisory Board by: 1) considering the feasibility of each recommendation; 2) discussing as <br />a team the opportunity for SAHA to implement each recommendation; and 3) making a determination on which recommendation(s) could be <br />implemented or considered for future implementation. One of the primary recommendations we received was that SAHA needs to assist the most <br />vulnerable populations in our community. A second primary recommendation was that the vacancy rate is very low in Santa Ana which prevents <br />participants from finding and locating units. The RAB recommended we conduct more landlord outreach. All of the other recommendations are <br />summarized in the comments and were taken into consideration by the agency. A decision was made on each recommendation and implemented in <br />the draft HCV Administrative Plan. <br /> <br /> <br />C.4 <br /> <br />Certification by State or Local Officials. <br /> <br />Form HUD 50077-SL, Certification by State or Local Officials of PHA Plans Consistency with the Consolidated Plan, must be submitted by the <br />PHA as an electronic attachment to the PHA Plan. <br /> <br /> <br /> <br />D <br /> <br />Statement of Capital Improvements. Required in all years for all PHAs completing this form that administer public housing and receive funding <br />from the Capital Fund Program (CFP). <br /> <br />D.1 <br /> <br />Capital Improvements. Include a reference here to the most recent HUD-approved 5-Year Action Plan (HUD-50075.2) and the date that it was <br />approved by HUD. <br /> <br />3-6
