CITY OF SANTA ANA <br />REQUEST FOR PROPOSALS <br />DEMOLITION OF THREE SINGLE - FAMILY DWELLINGS <br />1. INTRODUCTION <br />The City of Santa Ana is seeking a qualified demolition contractor to demolish three single - family dwellings <br />located at 710, 714 and 720 E. 6th Street in the City of Santa Ana. Work includes the removal of lead and <br />asbestos containing materials. <br />2. SCOPE OF WORK <br />The Scope of Work, attached as Exhibit A, must be signed and returned with the proposal. Proposals that do <br />not contain the signed Scope of Work will be considered non - responsive and rejected. <br />3. LICENSE REQUIREMENTS <br />• California Contractor's License — Proposers must possess a valid California Contractor's License in the <br />classification of C -21, Asbestos Certificate and be DOSH registered. Evidence of said license and <br />certificate shall be included in the proposal. Proposals that do not contain this information will be <br />considered non - responsive and rejected. <br />• City of Santa Ana Business License — The awarded individual or firm shall obtain a City of Santa Ana <br />Business License within 10 business days of the date of the notification of award. Failure to obtain said <br />license within the time prescribed above will be cause for cancellation of the award and selection of <br />another qualified individual or firm. <br />4. INSURANCE REQUIREMENTS <br />Insurance certificates evidence Insurance certificates evidencing the coverage described below shall be <br />included in the proposal. Proposals that do not contain this information will be considered non - responsive and <br />rejected. <br />• Commercial General Liability Insurance with a combined single limit of $1,000,000 per occurrence. <br />Successful bidder is required to add the City of Santa Ana as additional insured on a standard Commercial <br />General Liability Policy during the course of construction (see Attachment 3). <br />• Worker's Compensation Insurance with statutory limits of not less than $1,000,000 per accident. <br />• Business Automobile Liability Insurance with a combined single limit of not less than $1,000,000 per <br />occurrence. <br />5. BID BOND <br />Each proposal shall be accompanied by a bid bond issued by a corporate surety, for an amount not less than <br />ten percent (10 %) of the aggregate of the bid as a guarantee that the bidder will enter into the proposed <br />contract if the same is awarded to the bidder. Said bid bond shall be valid for 90 days from the bid closing <br />date. The signature of the bidder on the bid bond must be notarized.
