Scope of Services <br />IF Rapid Re- Housing (RRH) Program is designed to help chronically homeless families and individuals <br />from Santa Ana's Civic Center transition more rapidly out of the shelter system or street homelessness. <br />Our goal is to identify barriers and provide support to overcome barriers to success while stabilizing <br />families and individuals in permanent housing as quickly as possible. The Santa Ana ESG funds will <br />provide to individuals with permanent housing solutions along with individualized services to help <br />improve their social and economic well being. <br />The funds will provide: <br />- Housing Relocation and Stabilization for Santa Ana Civic Center homeless families and individuals <br />- Housing search, Placement, and Retention services <br />- Applications Fees, Deposits, Last Month's Rent, and Moving Costs <br />- Case Management and Wrap- around Services <br />- Credit Counseling /Financial Literacy <br />- Short to Medium -Term Rental Assistance <br />Case Managers will provide crisis stabilization and assess barriers to housing for each individual or <br />family. An individualized plan will be developed that empower and motivates clients to work towards <br />their goals. The client will be provided a comprehensive set of enabling services including life skill classes, <br />job readiness training and job placement services, medical and behavioral health care, substance abuse <br />counseling, benefits acquisition, and connection to other community resources. A housing specialist will <br />work with the client to identify housing needs and preferences, partners with landlord and assist with <br />locating viable housing. Housing specialists will assist with negotiating leases, and offer tenancy training <br />and retention services as necessary. <br />IF is interested in leveraging this year's RRH funds with our Recuperative Cares Post - Hospital Recovery <br />Program to provide housing stability for several of the Civic Center homeless individuals. Area hospitals <br />already send homeless individual who need further recovery after their hospital treatment to IF's <br />recuperative care centers. Here they receive medical oversight along with intense Case Management <br />services in a shelter setting for ro to 14 days, or more. Using RRH funds to provide permanent housing <br />solutions for the most medically vulnerable of the Civic Center homeless individuals will demonstrate a <br />good alternative strategy to the shelter for the homeless. <br />
