EMPLOYEE GROUP INSURANCE RENEWALS {STRATEGIC PLAN NO. 7, 6} - <br />Personnel Services Agency <br /> <br /> RECOMMENDED ACTIONS: <br /> <br /> <br />1.AGMT - Authorize the City Attorney to prepare and authorize the City Manager <br />and Clerk of the Council to execute agreements for the renewal of dental, vision, <br />employee assistance, retiree health savings plan, long term disability, employee <br />group life, and accidental death and dismemberment insurance, subject to non- <br />substantive changes approved by the City Manager and City Attorney. <br /> <br /> <br />2.Authorize the City Manager to approve the rates as offered by CalPERS for 2017 <br />Health Premiums for medical insurance. <br /> <br /> <br />3.Authorize the City Manager and Clerk of the Council to recognize the ongoing <br />agreement with REACH Employee Assistance, Inc., executed in January 1, 2015 <br />expiring on December 31, 2017, subject to non-substantive changes approved by <br />the City Manager and City Attorney. <br /> <br /> <br />4.Authorize the City Manager and Clerk of the Council to recognize the ongoing <br />agreement with ICMA-RC as the Plan Administrator of the VantageCare Retiree <br />Health Savings Plan through December 31, 2017. <br /> <br /> <br />25E. AGMT - COMPREHENSIVE MEDICAL SERVICES FOR CITY EMPLOYEES AND <br />PRE-EMPLOYMENT ASSESSMENTS FOR PROSPECTIVE EMPLOYEES <br />{STRATEGIC PLAN NO. 7, 5E} - Personnel Services Agency <br /> <br />RECOMMENDED ACTION: Authorize the City Manager and Clerk of the <br />Council to execute a first amendment with Occu-Med, Ltd .’s existing three year <br />agreement, July 1, 2015 - June 30, 2018, to increase the annual amount by $25,000 <br />with a new not to exceed annual amount of $100,000 and a new not to exceed <br />amount of $300,000 for the remainder of the term, subject to non-substantive <br />changes approved by the City Manager and City Attorney. <br /> <br /> <br />25F.
