The Original Agreement and this First <br />Amendment are and shall continue to be administered by the Health Care Agency <br />( "ADMINISTRATOR "). <br />WITNESSETI3 <br />WHEREAS, on February 1, 2016, COUNTY and CONTRACTOR previously entered into that <br />certain Agreement for the provision of Fifteen In Twenty (ITT) Cities Mini Grant Services for the period <br />February 1, 2016 through June 30, 2016; and <br />WHEREAS, COUNTY and CONTRACTOR now desire to amend the Agreement to extend the <br />term for the provision of Fifteen. In 'Twenty (FIT) Cities Mini Grant Services from June 30, 2016 to <br />August 31, 2016; <br />NOW, THEREFORE, IT IS MUTUALLY AGREED AS FOLLOWS: <br />1, Page 4, line 3 of the AGREEMENT is amended to read as follows: <br />"Term: February 1, 2016 through August 31, 2016" <br />Page 1, lines 24 -36 and Page 2 lines 1 -6 of the Exhibit A to the AGREEMENT are amended to read as <br />follows: <br />"II. PAYMENTS <br />A. COUNTY shall pay CONTRACTOR a negotiated rate of $30,000. Services, shall include but <br />not be limited to, covering travel, parking, gas, supplies, and any potential equipment use; provided, <br />however, the total of such payments shall not exceed the COUNTY's maximum obligation, as set forth <br />in the Referenced Contract Provisions of the Agreement. CONTRACTOR shall submit invoices for said <br />services and be reimbursed leased on the satisfactory completion, as determined by the <br />ADMINISTRATOR, of all six (6) of the performance objectives outlined in the Agreement. <br />CONTRACTOR will be required to submit Program Reports that include but are not limited to the hours <br />of services provided each week and such supporting documentation as ADMINISTRATOR may require. <br />1 of 4 <br />ViAPII K MGMT\PI1 VFNDott Fiu; (5)1FH- Cin Arrzia €MrMir '1 ,dNAI.
