Y <br />City of Santa Ana Response to Request for Proposal (RFP) Medical Services Review 2/9/15 <br />alternate job, for which the employee is otherwise qualified (retains the necessary knowledge, skills and <br />abilities), exists and is presently vacant, allowing the employee to return to work With the City under a <br />newjob title, If alternative job placement is not available, the client now has legally defensible <br />documentation tojustify that the proposed accommodations are unreasonable. Occu-Med's report (Med- <br />Legal Analysis) integrates the functional limitations of the employee and the legal and practical <br />ramifications regarding the agency's ability to accommodate the employee's restrictions or <br />the legally appropriate path for terminating the employee, Occu-Med considers all the relevant factors of <br />the case before rendering a medically appropriate and legally defensible job suitability recommendation. <br />In cases where the employee is unresponsive to OCCn-Med's correspondence a report documenting the <br />( attempts to contact the employee and the employee's failure to engage in the reasonable accommodation <br />P process will be provided to the City. With the City's approval,a Notification of Medical Disqualification <br />1 (Failure to Engage in the Reasonable Accommodation Process) letter will be sent to explain to the <br />I employee that his or her lack of response to the Individualized Assessment/Reasonable Accommodation <br />I invitation constitutes his or her Failure to engage in the reasonable accommodation process. This letter <br />I may also be used in the case that the employee's medical condition is a contraindication to job placement <br />I and no reasonable accommodation could be made. OCen-Med's IRTW Report and recommendation will <br />allow the City to make an informed, legally defensible decision as to whether or not an employee should <br />be returned to work. <br />E. Occ u. VA X <br />DCett-VAX is the scheduling and coordination, immunization record management and tate on-going <br />management of all immunization and chemoprophylaxis treatments for applicants and employees for jobs. <br />This service includes an individualized evaluation of each applicant/ employee to assess the <br />appropriateness and necessity for administering Che immunization and chemuprophyla:xis treatments <br />outlined in the specific OCOLI-VAX Profile prior to and, When appropriate, during employment. <br />Scheduling a nd C ordinatlon: Upon completion of the medical evaluation or concurrently by <br />requirement or request from a designated City employee, OCCa-Med will promptly contact applicants / <br />employees to: a) assess the Appropriateness and necessity For receiving immunization and <br />chemoprophylaxis treatments, and b) schOclule and coordinate the administration of those immunization <br />and chemoprophylaxis treatments deemed appropriate and necessary prior round during employment. <br />p�16Occu•Med m.w»„,.�„„,�-^*• <br />
