NOTE: Same agreement will be used for nonprofit <br />organizations approved for the CEPG program. <br />COMMUNITY ENHANCEMENT PROGRAM GRANT AGREEMENT BETWEEN <br />THE CITY OF SANTA ANA AND <br />This Community Enhancement Program Grant Agreement ("Agreement") is made and entered <br />this _ day of 2017, by and between the City of Santa Ana, a charter City and <br />municipal corporation organized and existing under the Constitution and laws of the State of <br />California ("City"), and Organization Name , a California section 1501(c)(3) Nonprofit <br />Organization ("Grantee"), collectively referred to herein as the "Parties". <br />RECITALS: <br />A. On October 14, 2016, applications for the Community Enhancement Program Grant (CEPG) <br />were released. <br />B. On December 6, 2016, the City Council approved the recommended Fiscal Year 2016-2017 <br />CEPG Grants, thereby allocating $500,000 to nonprofit organizations for public services that <br />support health, human and/or quality of life services to Santa Ana residents. <br />C. On December 6, 2016, the City Council reviewed all applicants and determined that Grantee <br />is hereby awarded a CEPG based on its Application for its public services ("Project') attached <br />hereto and incorporated herein as Exhibit A, and authorized the execution of this Agreement. <br />D. In undertaking the performance pursuant to this Agreement, Grantee represents that it is skilled <br />and knowledgeable in the public services arena and that any Project performed hereunder will be <br />performed in compliance with such standards as may reasonably be expected from a service <br />provider. <br />NOW THEREFORE, in consideration of the mutual and respective promises, and subject to the <br />terms and conditions hereinafter set forth, the parties agree as follows: <br />1. Grant Activities. Grantee agrees: <br />1.1. To perform the activities described in the Grant Application and Timeline submitted to City <br />for consideration dated , 2017, a copy of which is attached as Exhibit A and <br />incorporated into this Agreement as if set out in full. Service levels in the application may be <br />reasonably adjusted at the sole discretion of the City, proportionate to the level of funding <br />awarded. <br />1.2. To submit two reports (each, a 'Report'), one at the midway point of the Project, and one at <br />the end of the Project, as reasonably determined by the City. The Reports shall include, at a <br />minimum, the total number of Santa Ana residents served, a description of activities and <br />accomplishments, and financial documentation to support expenditures, such as true copies of <br />invoices, receipts, canceled checks, bank statements, credit card statements, payroll records, <br />contracts or documentation pertaining to costs for subcontractors, and/or other documentation <br />supporting and evidencing how the CEPG funds were expended during the applicable reporting <br />period. If necessary, the City, in its sole discretion, may request additional reports and/or <br />information from the Grantee regarding the Project. No personally identifiable information shall <br />be included in any of the Reports, except where specifically requested. The Reports shall be in <br />a format that is reasonably acceptable to City. City may request additional information as City, in <br />its sole discretion, determines necessary to monitor performance of this Agreement. City shall <br />have the right to use any Reports submitted by Grantee, or any portion thereof, for any reason. <br />
