Relationship Management: This section of the proposal must include a fully - <br />completed copy of the Summary Sheet included with this RFP (Attachment 4). <br />1. Provide names, titles, telephone numbers, fax numbers, and e-mail <br />addresses, and provide brief biographies of Bank relationship management <br />personnel. Identify primary and secondary representatives assigned to the <br />City's accounts. Describe what each person's role and responsibilities will be. <br />2. Provide resumes of the Bank Management Contact with the City and the <br />Project Manager. <br />3. Describe how the Bank plans to handle problem resolution, customer service, <br />day -to -day contact, and ongoing maintenance for the City of Santa Ana. <br />Describe the Bank's organizational structure as it relates to governmental <br />clients and any other information that will be helpful in understanding the <br />Bank's internal relationships. <br />D. Allocation of Resources <br />Provide a conceptual plan for services to the City that your Bank believes are <br />appropriate for the City. Indicate features, skills and /or services which distinguish <br />your Bank and make it the better choice for the City. Indicate how the resources of <br />your Bank (e.g., number and type of personnel allocated by hours) will be allotted <br />for this project. Submittal of a project schedule is required as part of the <br />Allocation of Resources. <br />E. Bank Compensation (Fees) <br />Submit your Bank's pricing proposal indicating all fees that would be charged to the <br />City. Fee proposals must be submitted using the City's format to be considered in <br />the selection process. A mandatory °COST /BID PROPOSAL FORM & <br />ATTACHMENTS are shown in Attachments 6, 6A & 6B. <br />F. Scope of Work and Time of Performance <br />Proposals must address all items set forth in Section 111 "Scope of Work and Time <br />of Performance." Additional information that your Bank wishes to include must be <br />clearly identified. The items must be addressed in the order in which they appear in <br />Section 111 of this RFP. <br />G. Client References <br />Each firm must include the following references: <br />1.
