July 1, 2017 -June <br />30, 2020 salary costs are based on total hours worked and clays the city <br />offices are closed for Workforce Specialist II and Fiscal Specialist <br />positions. <br />(2) Employee Benefits include contributions to 401k or retirement plans; <br />health insurance; dental insurance; life insurance; long -term disability <br />insurance; payroll taxes such as FICA, Federal Unemployment Tax, State <br />Unemployment Tax, and Workers' Compensation Tax, based an the currently <br />prevailing rates; and vacation and sick leave accrual limited to the <br />amount of time earned during the fiscal years in which such expense is <br />claimed. The overall benefit rate shall not exceed 55.0023% of the actual <br />salary expense claimed. <br />(3) Enrollments in WEX activities are contingent upon availability of funds. <br />(4) Benefits costs included are payroll taxes such as FICA, Federal <br />Unemployment Tax, State Unemployment Tax, and Worker's Compensation Tax, <br />based on the currently prevailing rates. <br />(5) Enrollments in VTR activities are contingent upon availability of funds. <br />15,2 CONTRACTOR and ADMINISTRATOR may agree, subject to advance written <br />notice, to add, delete, or otherwise modify line items and /or amounts and /or <br />(WRR0216) <br />Page 17 of 26 <br />25A -62 <br />(April 4, 2017) <br />
