MEMORANDUM <br />Fred Mousavipour <br />To: Executive Director- PWA <br />Tyrone Chesanek <br />From: Principal Civil Engine <br />Date: June 6, 2017 <br />Subject: United Storm Water Agreement A-2015-089 —Agreement Extension <br />Agreement A-2015-089 was entered by United Storm Water, Inc., and the City, on May <br />19, 2015, for the installation of Connector Pipe Screen (CPS) Units in City catch basins. <br />The terms of the Agreement were from May 19, 2015 through June 30, 2016 and the total <br />amount to be paid per this Agreement is not to exceed $201,600. <br />The Agreement may be extended by a writing executed by the City Manager and the City <br />Attorney and on May 18, 2016 the City extended the Agreement from July 1, 2016 <br />through June 30, 2017. <br />A consultant for the City is currently conducting a detailed analysis of the optimal <br />locations to install the CPS Units. Therefore, an additional extension of the Agreement is <br />required through June 30, 2018. This Agreement Extension will be the 2°a and final <br />extension of Agreement A-2015-089. <br />This Agreement packet includes a CAO -signed Contractor Agreement Extension, the <br />original Agreement A-2015-089, the first Agreement extension A-2015-089-01, and the <br />approved insurance certificate. <br />Please initial and approve for routing to City Manager and Clerk of the Council. <br />
