In such event, Plan Sponsor shall also be responsible for providing dis <br />to Medicare Part D eligible individuals who are covered under or apply for covt <br />Plan advising whether the prescription drug coverage provided through the Plan, <br />alone basis or combined with another group health plan, is creditable. <br />2o.
