RECEIVE AND FILE EMPLOYEE HEALTH INSURANCE RATE RENEWALS FOR <br />CALENDAR YEAR 2018 AND EMPLOYEE DENTAL, LIFE AND LONG-TERM <br />DISABILITY, AND VISION INSURANCE FOR CALENDAR YEARS 2018-2019; <br />APPROVE AN AGREEMENT FOR EMPLOYEE ASSISTANCE PLAN SERVICES <br />AND FOR MANAGEMENT OF FLEX SPENDING ACCOUNTS AND RETIREE <br />DENTAL BILLING SERVICES; AND, RECEIVE AND FILE CONTINUED <br />AGREEMENT WITH ICMA-RC VANTAGECARE RETIREMENT HEALTH <br />SAVINGS (RHS) PLAN {STRATEGIC PLAN NO. 7, 6} - Personnel Services Agency <br /> <br /> RECOMMENDED ACTIONS: <br /> <br /> <br />1.Receive and file rates for employee health insurance through CalPERS for <br />calendar year 2018. <br /> <br /> <br />2.Receive and file rates for employee dental insurance with Delta Dental and <br />MetLife, employee life and long-term disability insurance coverage through <br />Aetna and employee vision insurance through EyeMed for the calendar years <br />2018 and 2019. <br /> <br /> <br />3.Authorize the City Manager and Clerk of Council to execute an agreement with <br />REACH to provide employee assistance plan services from January 1, 2018 to <br />December 31, 2020 at a monthly cost of $1.85 per full time employee and $1.25 <br />per part-time employee, subject to non-substantive changes approved by the <br />City Manager and City Attorney (AGMT.
