S. <br />The Consolidated Five -Year Plan (Plan) is a comprehensive planning and application document <br />that covers the three entitlement grants that the City receives annually from the U. S. Department <br />of Housing and Urban Development (HUD). The Five -Year Plan outlines a strategy for using the <br />three grants and other resources to meet community needs as they were identified in the Plan. <br />These three entitlement grants are Community Development Block Grant (CDBG), Emergency <br />Solutions Grant (ESG), and HOME Investment Partnerships Program (HOME). The Five -Year <br />Plan also identifies five-year numeric goals for meeting the identified community needs that Santa <br />Ana will work toward during the period covered. <br />As part of the Consolidated Plan process, the City is required to annually submit a Consolidated <br />Annual Performance and Evaluation Report (CAPER) (Exhibit 1). The CAPER describes the <br />activities and accomplishments for these federal entitlement grants during Fiscal Year 2016-2017. <br />It also reports the City's success in achieving the five-year goals and objectives that are <br />established in the Five -Year Plan. Fiscal Year 2016-2017 is the second year covered by the <br />2015/16 through 2019/20 Consolidated Five -Year Plan. <br />19D-1 <br />
