a h are <br />your independem healthcare choiw. <br />team works closely with local mental health agencies to effectively manage inmates and plan for <br />their care after release. <br />NaphCare's mental health services are built on a foundation of quality staff that focus on: <br />✓ Proactive identification of persons with behavioral health issues, <br />✓ Stabilization, <br />✓ Continued monitoring, <br />✓ Rapid crisis intervention, <br />✓ Effective programming, <br />✓ Constant, clear and effective communication with facility security leadership and staff, <br />and <br />✓ Collaboration with community mental health providers at discharge. <br />NaphCare's Chief Medical Officer, Dr. Steven Bonner, will continue to be available for clinical <br />support and oversight of the SAJ's mental health program. He has over 16 years of clinical <br />experience as a Board -Certified Psychiatrist. He serves as a member of NaphCare's Morbidity & <br />Mortality Committee, and he chairs the Pharmacy & Therapeutics Committee. He also performs <br />peer reviews, and provides provider oversight and clinical support to all NaphCare mental health <br />programs. <br />NaphCare will provide mental health training services to mental health professionals and <br />correctional staff on such topics as symptom recognition, referral, and de-escalation techniques. <br />Ourgoal is to reduce recidivism by collaborating with community mental health resources for <br />effective discharge planning. <br />Mental Health Preliminary Screening and Evaluation <br />NaphCare's first opportunity to identify a patient with mental illness is during the Intake <br />Screening. Our Receiving Screen asks about medical and mental health issues to proactively <br />identify patients with these problems as soon as they arrive. Following the Receiving Screen, a <br />Mental Health Screen is performed per NCCHCJ-E-05, Mental Health Screening and Evaluation, <br />and NaphCare policy and procedure. All inmates will receive a mental health screening, to <br />include a suicide risk assessment, by a trained nurse upon arrival. <br />The following is a description of each section of NaphCare's Mental Health Screening and the <br />information it obtains: <br />• Current Mental Health Symptoms — Reviews depressed mood, anxiety, psychosis, and <br />mania. In addition, inquiries are made regarding current mental health treatment in the <br />community, any suicidal thoughts, any recent losses, feelings about current situation, if <br />they feel they have anything positive in their future, and allows the interviewer to <br />comment on their feeling of suicide risk based on responses and appearance. <br />Inmate Medical Services <br />City of Santa Ana <br />Past Mental Health History— Reviews past treatment for mental health issues including <br />medications and hospitalizations; reviews history of self -injury behaviors and suicide <br />attempts. <br />25F-48 <br />RFP No. 17-051 <br />49 <br />
