a h are <br />your independent heolthcom choice. <br />• Substance Abuse — Inquires about alcohol, benzodiazepine, opiate, and other substance <br />use issues, as well as history of substance use related treatment. <br />PREA/General Assessment— Reviews any history of abuse of any kind, in any setting; <br />any convictions for sex or violent crimes; any history of special education or <br />developmental disabilities; history of head injury or seizures. <br />• Disposition/Treatment Plan — Allows the interviewerto refer to indicated mental health <br />services, refer for detox services if indicated, and/or begin discharge planning with <br />regard to need for mental health follow up. <br />NaphCare is committed to identifying and meeting the acute and chronic mental health needs of <br />patients admitted to the SAJ. Each patient who is identified by the screens as needing mental <br />healthcare treatment will have action taken to ensure this care is provided. Any inmate who is <br />determined to need additional mental health services will be scheduled for further evaluation <br />by higher level mental health professionals (up to and including psychiatric evaluation) in the <br />clinically indicated time frame. <br />In TechCare® we electronically flag an inmate's health record if his/her responses during intake <br />indicate the need for additional mental healthcare. All screenings and evaluations will identify <br />inmates with suicidal and homicidal tendencies, as well as acute and chronic behavioral health <br />issues. Alerts in the system prompt the clinical staff to take action, such as placing an inmate on <br />suicide precautions, contacting custody about an issue, or assigning special housing. The Mental <br />Health Screen also contains prompts to assist the interviewer in taking any indicated actions <br />such as suicide watch, or urgent mental health referral based on the inmate's responses. All <br />mental health encounters are documented electronically in the inmate's health record in <br />TechCare®. The mental health provider can access the inmate's entire electronic health record <br />when conducting an evaluation, or at any time the inmate is referred to the mental health <br />program during his/her incarceration. <br />Additionally, if or when an inmate is transferred to another facility, his or her health record, <br />containing mental health information, will be instantly available in TechCare®. This ensures <br />continuity of care for inmates with a mental illness. <br />Proactive Quality Assurance Checks <br />✓ Using TechCare® we audit the initial mental health screenings. TechCare® searches the <br />database of all active inmates with a bed assignment for completed mental health <br />screenings. Corporate staff then reviews the screening report Monday through Friday, <br />and any missed screenings are reported to the site Health Services Administrator (HSA). <br />The HSA follows up on all identified inmates, and these inmates receive a mental health <br />screening within 48 hours of site notification. <br />✓ Per NCCHC standards, any inmate with a positive mental health screening requires a <br />mental health evaluation. Weekly, we run a TechCare® report showing all active <br />inmates who had a positive mental health screening, and have not had a mental health <br />evaluation yet.
