FRAZIER Approved By: <br />Date of Report: 06/22/2015 Date: 07/08/2015 <br />Page 1 of 4 <br /> <br /> <br /> <br />CASE NAME: OSORIO, BENITO <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) <br />TYPE: CANVASS <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br /> On Wednesday, March 11, 2015, at approximately 1340 hours, Orange County District Attorney <br />Supervising Investigator MARK GUTIERREZ contacted me regarding an officer involved shooting incident <br />involving the Santa Ana Police Department. GUTIERREZ requested I respond to the incident location, in <br />the city of Santa Ana, and assist with the investigation. <br /> At 1424 hours, I arrived at the incident location and met with investigators from the Orange <br />County District Attorney’s Office and from the Santa Ana Police Department who were present to assist <br />in the investigation. <br /> At 1513 hours, Santa Ana Police Sergeant JEFF SMITH conducted a briefing of the known <br />circumstances surrounding the incident for the personnel involved in the investigation. Following the <br />briefing, Orange County District Attorney Supervising Investigator MARK GUTIERREZ assigned Santa Ana <br />Police Detective JIM GARCIA, a certified Spanish language interpreter, and I to conduct canvass <br />interviews of potential witnesses that live in the area. During our initial canvass, GARCIA and I <br />conducted audio digital recorded interviews of the following individuals: <br /> <br /> <br /> told us in Spanish that he was in the rear of his residence, in the restroom, when he <br />heard sirens. looked out his living room window that faced Main Street, and saw a white pick- <br />ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE <br />BUREAU OF INVESTIGATION <br />SUPPLEMENTAL REPORT
