Kelly Approved By: <br />Date of Report: 9/10/12 Date:09/10/12 <br />Page 1 of 2 <br /> <br /> <br />ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE <br />BUREAU OF INVESTIGATION <br /> <br />SUPPLEMENTAL REPORT <br /> <br />CASE NAME: ALMANZA, MARCELINO VALENCIA <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON‐FATAL) <br />TYPE: HOSPITAL <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br /> <br /> On Monday, September 10, 2012, at approximately 0102 hours, Orange County District <br />Attorney Supervising Investigator RON SEMAN contacted me regarding an officer involved <br />shooting incident involving the Santa Ana Police Department (SAPD). SEMAN requested I <br />respond to University of California, Irvine Medical Center (UCIMC), located at 101 The City <br />Drive, in the city of Orange, and conduct investigative responsibilities at the hospital. <br /> At approximately 0152 hours, I arrived at UCIMC Emergency Room (ER). I met with ER <br />Registered Nurse (RN) outside of the operating room where she had just <br />delivered the patient for surgery. advised the patient was under the name of DOE, <br />YARD with a medical record number of and his given name was MARCELINO <br />ALMANZA. I requested place a hold on DOEs pre‐transfusion blood. <br /> took me to the operating waiting room where I met SAPD Patrol Officer <br />DANIEL ALCALA. We were later me by SAPD Corporal Detective DEAN FULCHER. ALCALA <br />explained that ALMANZA was transported to UCIMC via ambulance and ALCALA followed in his <br />patrol car.
