Agreements with AppleOne, HB Staffing, and Helpmates; and Processing of Staffmark Invoices <br />June 5, 2018 <br />Page 2 <br />within four (4) hours of the initial request, the City reserves the right to contact other temporary <br />agencies for placement. <br />• Once a candidate is selected and the hourly pay rate has been agreed upon, the Contractor(s) <br />shall provide the City with a detailed proposal to include the specific job description, the hourly <br />pay rate for the temporary worker, and the mark-up rate. The City will then sign the proposal <br />in approval. All invoices must include the agreement number. <br />• The Contractor(s) shall provide temporary workers who have the skills required for the <br />assignment. <br />• The City reserves the right to end a temporary staffs assignment if work performed is <br />substandard, attendance is poor, or for other business related reasons. The Contractor(s) <br />shall remove any temporary worker from their assignment at the City's request, and at the <br />time requested by the City, and a provide replacement temporary worker if required by the <br />City. <br />• The Contractor(s) shall be responsible for all decisions concerning matters of hiring, firing, <br />discipline, payroll practices, and employee benefits such as vacation, sick and other leave <br />benefits. Temporary workers provided by the agency shall be considered employees of the <br />agency and shall in no sense be considered employees or agents of the City. <br />• The Contractor(s) shall be responsible for communication with temporary workers with respect <br />to their duties, as employees of the Contractor(s). <br />• The Contractor(s) will provide the City with testing resources for regular status positions as <br />requested. These services shall be provided at the rates set forth in the Proposal Pricing <br />Sheet on Page 16. <br />• Individual temporary workers are not permitted to work more than 960 hours in any fiscal year <br />period (July 1 — June 30). <br />The temporary staffing services will focus on the following City staffing needs: general <br />administrative office support, executive / senior administrative office support, accounting and <br />financial administrative support, legal secretary support, and other classifications as determined. <br />This RFP resulted in 14 proposals from various staffing agencies. AppleOne, HB Staffing, and <br />Helpmates were selected. Proposer evaluation scoring results are attached. <br />The Clerk of the Council Office is also requesting authorization to pay outstanding invoices with <br />Staffmark in an amount not to exceed $5,150.52 for temporary. staffing services rendered in <br />February through the end of March of 2018. <br />251-2 <br />
