Consolidated Annual Performance Evaluation Report <br />October 2, 2018 <br />Page 2 <br />The CAPER describes the activities and accomplishments for these three federal entitlement <br />grants during Fiscal Year 2017-2018 (Exhibit 1). It also reports the City's successes in achieving <br />the five-year goals and objectives that are established in the Five -Year Plan. Fiscal Year 2017- <br />2018 is the third year covered by the 2015/16 through 2019/20 Consolidated Five -Year Plan. <br />The following table highlights the accomplishments achieved during Fiscal Year 2017-2018: <br />Activity <br />Accomplishments <br />• Completed the Pacific Electric Community Park, which included a <br />$2.7M investment of CDBG funds towards Santa Ana's newest <br />park. The park features open space for the community that <br />includes native plant gardens, playgrounds, walking space, a <br />fitness area, and gardening beds. <br />• Completed Roosevelt Walker Community Center, which included a <br />$2.11M investment of CDBG funds as a joint -use project with the <br />Capital <br />Santa Ana Unified School District. The site includes a 10,000 sq. ft. <br />Improvements using <br />community center, basketball courts, play equipment, exercise <br />CDBG Program <br />equipment, community garden, parking lot, security lighting, and <br />Funds <br />landscaping. <br />• Completed Madison Residential Street Improvement, which <br />included a $598,053 investment of CDBG funds used to increase <br />accessibility and sustainability of Santa Ana streets. The activity <br />consisted of the replacement of pavement on various residential <br />streets within the City of Santa Ana utilizing a cement treated base <br />process. The activity included construction of asphalt overlay, <br />replacement of concrete curbs, curb ramps, gutter, and sidewalk, at <br />various locations within the City of Santa Ana. <br />• Funded 16 programs and served 3,127 persons including: <br />o Youth services provided by the Illumination Foundation, <br />America on Track, Boys & Girls Club of Santa Ana and OC <br />Children's Therapeutic Arts Center, Neutral Ground, Big <br />Brothers Big Sister of OC, YMCA and Albi included tutoring, <br />education, recreation, fitness, and a classical music program. <br />o Youth and family services provided by the Cambodian Family <br />Public Services <br />and the Delhi Center included tutoring, education, leadership, <br />using CDBG <br />community beautification, parent workshops, and anti -bullying <br />Program Funds <br />workshops. <br />o Criminal Record Expungment and Domestic Violence legal <br />services provided by the Legal Aid Society of Orange County. <br />o Homeless Services provided by WISEPlace included shelter <br />and supportive services to homeless women. <br />o Access to health and social service programs by outreach, <br />education and enrollment, provided by Community Health <br />Initiative of OC. <br />75A-2 <br />
