PROPOSAL TO PROVIDE FIRE/EMS EMERGENCY TRANSPORTATION & RELATED SERVICES <br />CITY OF SANTA ANA <br />Doctor's Ambulance headquarters are located at 23091 Terra Drive, Laguna Hills, California <br />92653. There is no defined expiration date to the agreement. <br />Emergency Ambulance <br />Emergency is a current contracted OCFA BLS ambulance service that meets all OCFA <br />operational requirements, providing emergency ambulance services to the communities of <br />Brea, Placentia and Yorba Linda. Emergency Ambulance uses a crew staffing of two (2) EMTs <br />and has fifteen (15) Orange County licensed ambulances. Emergency Ambulance began service <br />in 1977 and is CAAS Accredited. <br />Emergency Ambulance headquarters are located at 3200 E. Birch Street, Suite A, Brea, <br />California 92821. Contact person is Chad Drutten — Chief Operating Officer. Telephone number <br />is (714) 990-1331. There is no defined expiration date to the agreement. <br />Shoreline Ambulance <br />Emergency is a licensed Orange County EMS provider and former OCFA contractor to the City of <br />Westminster. Currently, Shoreline Ambulance maintains a non -emergency ambulance station <br />and crew in the City of Santa Ana. Shoreline Ambulance uses a crew staffing of two (2) EMTs <br />and has seven (7) Orange County licensed ambulances. <br />Shoreline Ambulance headquarters are located at 3400 W. Warner Avenue, Suite H, Santa Ana, <br />CA 92704. Contact person is Jim Karras— Chief Operating Officer. Telephone number is (855) <br />474-6735. There is no defined expiration date to the agreement. <br />Regional Disaster Transportation Coordination Center <br />In addition to being one of the largest providers of emergency ambulance services in Southern <br />California, Care Ambulance also serves as the Regional Disaster Transportation Coordination <br />Center for our Southern California Region. From localized disasters to state wide disasters, Care <br />Ambulance provides the required emergency ambulance coordination for these events. <br />Care's Regional Disaster Transportation Coordination plan and copies of our mutual aid <br />agreements are provided as an Attachment to this section of the proposal. <br />RFP # 18-059— CARE AMBULANCE SERVICE 17 <br />
