City of Santa Ana <br />A. Be filed in writing within five (5) business days after the RFP issue date or before <br />5:00 p.m. of the 5th business day following the posting of RFP Results/Notice of <br />Intent to Award Contract on the City's website. <br />B. Clearly identify the specific irregularity or accusation. <br />C. Clearly identify the specific City staff determination or recommendation being <br />protested. <br />D. Specify, in detail, describing the grounds of the protest and the facts supporting <br />the protest, and <br />E. Include all relevant, supporting documentation with the protest at time of filing. <br />If the protest does not comply with each of these requirements, it will be rejected as <br />invalid. If the protest is valid, the City's Purchasing Manager, or other designated City <br />staff member, shall review the basis of the protest and all relevant information. The <br />Purchasing Manager will provide a written decision to the protestor within ten (10) <br />business days from receipt of the protest. The decision from the Purchasing Manager, <br />or designee, is final and no further appeals will be considered. <br />5.0 PROPOSAL FORMAT GUIDELINES: <br />Interested entities or contractors are to provide the City a thorough Proposal using the <br />following guidelines: Proposal should be typed using a 12-point font size, and should <br />contain no more than 100 typed pages (including cover letter, required Attachments `A" <br />through "F", and resumes of key personnel, Index/Table of Contents, dividers, tables, <br />charts, graphic exhibits, and costs proposal). Each Proposal will adhere to the following <br />order and content of sections: proposal should be straightforward, concise and provide <br />"layman" explanations of technical terms that are used; emphasis should be <br />concentrated on conforming to <br />requirements, and on providing <br />City of Santa Ana <br />Ambulance & Paramedic and Collection Services <br />Request for Proposals: # 19-083 <br />September 13, 2019 <br />the RFP instructions, responding to the RFP <br />a complete and clear description of the offer. <br />25B-35 <br />Page 112 <br />
