Additionally, Human Options <br />shall complete a FaCT standardized Counseling Assessment Tool and the FaCT <br />standardized Mental Health Counseling Indicators Checklist. <br />3.6.8 Human Options' DR Family Counseling services shall address <br />the following PSSF service category: Family Preservation. <br />3.6.9 Human Options shall provide qualified DR Counselor and <br />Clinical Supervisor staff as specified in Subparagraphs 14.14 and 14.15 of <br />this Exhibit. <br />3.7 DR Crisis Counseli <br />3.7.1 Human Options shall provide DR Crisis Counseling services <br />to at -risk parents, caregivers, and/or their children ages birth to eighteen <br />(18) years of age in the community who have been identified by ADMINISTRATOR <br />and referred to CONTRACTOR as potential risks for child abuse and/or neglect <br />and/or referred by DR Family Advocate staff, hereinafter referred to as <br />"PARTICIPANTS" for purposes of Subparagraph 3.7 of this Exhibit. <br />3.7.2 Human Options shall provide DR Crisis Counseling services <br />to a minimum of thirteen (13) PARTICIPANTS; DR Crisis Counseling services <br />shall include, but not be limited to: assessing family needs: providing <br />treatment planning; providing emotional support; stabilizing immediate crisis; <br />increasing PARTICIPANTS' coping skills and family coherence; reducing <br />PARTICIPANTS' exposure to violence in the home; providing assistance <br />developing time -limited goals for the family and the child in placement that <br />(F*10209x <br />13 of 42 <br />(3/18/09) <br />
