the Affordable Care Act (ACA), through another source other than coverage in the <br />individual market (whether or not obtained through Covered California). <br />10.2 Dentallnsurance <br />A. The City offers two (2) types of dental insurance plans. One (1) is a Dental Health <br />Maintenance Organization (DHMO) plan and one (1) is a Preferred Provider <br />Organization (PPO) plan. <br />B. The City's monthly contribution for dental insurance shall be based on the plan <br />coverage chosen by the employee. The City shall contribute seventy-five percent <br />('75%) of the City's contribution allotted to frill -time SEW represented employees <br />for dental insurance premium costs for up to the family (employee and all <br />dependents) coverage. <br />C. Any contribution necessary to maintain benefits under said dental plans in excess <br />of the amounts set forth above shall be home entirely by the employee. <br />10.3 Long -Term Disability Insurance <br />The City shall allow employees, for the term of this MOLT, to participate in any aspect of <br />the plan offered to the City at the City rate. The cost for this benefit shall be home by the <br />employee. <br />10.4 Life Insurance <br />The City shall allow employees, for the term of this MOU, to participate in any aspect of <br />the plan offered to the City at the City rate. The cost for this benefit shall be home by the <br />employee. <br />10.5 Benefits Advisory Committee <br />When there is a need to discuss matters relating to employee insurance benefits and the <br />City believes it would be beneficial to involve a Benefits Advisory Committee (BAC), each <br />party (the Union and the City) shall have an equal number of representatives on such a <br />committee to meet as necessary. <br />t0.6 Retirement Health Savings (RHS) Plan <br />Employees participate in the ICMA-RC "Vantage Care" RHS Plan subject to the <br />following: <br />� l <br />2019-2022 MOU City of Santa Ana & SEW Part -Time Civil Service Employees Un' ge 48 (-_(1 I SAY <br />r ( <br />�P <br />� <br />20C-51 <br />�rlI v <br />
