b. Understanding of Need <br />Proposal shall include an outline, which demonstrates the firm's understanding of <br />the scope of services and relevant experience performing relative tasks. This outline <br />should include anticipated approach, tasks necessary for successful completion, and <br />suggestions or special concerns that the City should be made aware of <br />c. Financial Capacity and Availability <br />To demonstrate a solid financial capacity and availability, the proposer shall <br />include a list of four (4) relevant positive bond ratings demonstrating financial <br />stability and reliability along with a list of past projects for which the company has <br />reliably been able to respond to an agency's on -call and/or emergency needs within <br />the last five (5) years. Project information should include project description, year <br />completed, client name, along with a person to contact and their telephone <br />number/email address. <br />d. References <br />The Proposer shall submit a list of agencies, past and present, for whom you have <br />provided work similar to that identified in this REP (Attachment 1: Scope of <br />Service) for the last five (5) years. <br />2. Fee Schedule <br />Proposer's fee schedule shall be submitted concurrently with the technical proposal, <br />but in a separate submission to the PlanetBids system. This shall include the firm's (and <br />any subcontractors') Standard Hourly Fee Schedule as stated in the Appendix: <br />Attachment 3: Fee Schedule. <br />Fees are NOT included as part of evaluation criteria, the fee proposal will not be opened <br />until the proposals have been evaluated by the proposal selection committee. The City <br />will select the Contractor based on qualifications, and then negotiate a contract price <br />based on available funding per work order task. <br />3. Certifications <br />The following forms, included in Attachment 4: Certifications in the Appendix of <br />this RFP, shall be signed and included as part of the proposal submittal package: <br />• Contractor's Licensing and Registration Statement <br />• Prevailing Wage Compliance and Monitoring Statement <br />• Ownership Affidavit <br />• List of Sub -contractors (If Included) <br />• References <br />• Non -Collusion Affidavit <br />• Non -Lobbying Certification <br />• Non -Discrimination Certification <br />• Statement Regarding Apprenticeship Requirements <br />City of Sant(@FF"106 <br />
