' 1, �.Ai,.. �Rk� <br />2215 N Broadway • Santa Ana, CA 92706 • 714.547.5468 • Fox 714.564.9690 • www.occtac.org <br />No replacement bus pass, gas card or ACCESS coupon book will be issued in the <br />event the participant loses his/her bus pass, gas card or ACCESS coupon book. <br />Fingerprinting —Live Scan and TB Test: <br />Participants who work with children and are placed in an educational setting will be <br />required to undergo Fingerprinting —Live Scan through the Department of Justice <br />and show a cleared T13 "hest. This supportive service will be available for participants <br />on a case -by -case basis and determined by the case manager_ <br />Work and School Related Materials: <br />A participant may need to purchase books, fees, school supplies and/or other <br />necessary materials to enable hirn/her to participate in activities leading to a <br />credential. A participant may need to purchase work related equipment, attire, tools, <br />or certification for the purpose of obtaining employment and to enable him her to be <br />successful in employment. <br />Laptops and 'fechnoloiy! During this time where Covid-19 is impacting program <br />services, if a need for technology such as a laptop, tablet, or wi-fi/hotspot is <br />determined in order to complete a W10A program goal, a participant may be eligible <br />for this supportive service on a case -by -case basis. This service is contingent upon <br />approval by the Program Coordinator and OCCTAC Director. (Fellow mk),o-purr hale <br />procedures oudlined in WSD 17-08 when purchasing xhese items.) <br />Child/Dependent Care <br />After identifying the need for assistance and eliminating other resources, the case <br />manager and the participant will develop a service plan (ISS) and update progress in <br />the case notes. <br />Childcare assistance is defined as the provision of $75 payment to <br />institution/dayeare/childcare provider upon receiving an invoice. <br />Participants may receive assistance with childcare or dependent care, which will <br />enable them to participate in activities. Childcare will be limited to a maximum of <br />$75.00 per month per child/dependent. Supportive service available to all youth that <br />enrolled in the program as pregnant or parenting youth. <br />Other: Any other miscellaneous type expenses not mentioned above may be considered with <br />proper authorization.
