v. Any relevant photographic or documentary evidence submitted to <br />support the allegations in the complaint; and, <br />vi. The identity of any witnesses, if known. <br />c. Complaint reports, pursuant to Section 5.2.2 of this Agreement. <br />d. Copies of all review reports submitted to the City pursuant to Section <br />5.2.2of this Agreement, which shall include at a minimum: <br />i. The complaint as received; <br />ii. The date the Contractor investigated the complaint; <br />iii. Documentation of the findings of the investigation; <br />iv. Any photographic or other evidence collected during the <br />investigation; and, <br />v. Contractor's recommendation to the City on whether or not the <br />entity investigated is in violation of SB 1383 based on the <br />Contractor's investigation. <br />16. Customer Waivers. Contractor shall provide a report that documents each <br />Customer waiver request reviewed by Contractor, which are required by <br />Section5.6.13. Identify in the report the Customer name and service address, the <br />type of waiver requested, the status of the waiver (accepted, denied, pending), <br />and other information reasonably requested by the City. <br />17. Solid Waste Evaluation Reports. In accordance with Section 8.5 of Exhibit 8, <br />Contractor shall provide reports of evaluations of Solid Waste conducted at <br />Approved Facilities, as applicable. <br />8.3.4 Annual Report <br />Annual reports shall be submitted within sixty (60) days following the reporting year. <br />The Annual Report is to be essentially in the form and content of the monthly and <br />quarterly reports combined, but shall also include: <br />1. Processing Facility Report <br />A.
