include the following documents and information: <br />a. The number of days the Processing Facility temporary equipment <br />waiver or operation failure waiver was in effect; <br />b. Copies of any notifications sent to the City and copies of City notices to <br />Contractor, pursuant to Section 4.9.3.A of the Agreement; <br />c. Documentation setting forth the date of issuance of the waiver and, the <br />timeframe for the waiver; and, <br />d. A record of the tons of Source Separated Recyclable Materials, Source <br />Separated Organic Materials, and/or Refuse redirected to an <br />Alternative Facility or Disposed at a Designated Disposal Facility as a <br />result of the waiver, recorded by Collection vehicle or Transfer vehicle <br />number/load, date, and weight. <br />B. Quarantined Organic Waste: A record of all compliance agreements for <br />quarantined Organic Waste that are Disposed of, including the name of <br />Customer, date issued, location of final disposition, and the amount of <br />quarantined Organic Waste that was required to be Disposed at a landfill. <br />C. Compostable Plastics in Source Separated Organic Materials Containers: <br />Written notification that the Approved Organic Waste Processing Facility <br />has and will continue to have the capabilities to Process and recover the <br />Compostable Plastics included with the Source Separated Organic <br />Materials Transported to the Approved Organic Waste Processing Facility. <br />D. Plastic Bags in Source Separated Organic Materials Containers: Written <br />notification to the City that the Approved Organic Waste Processing <br />Facility has and will continue to have the capabilities to Process and <br />recover plastic bags when it recovers Source Separated Organic Materials. <br />E. Any other Approved Facility incidents as deemed necessary to comply <br />with Section 4.9. <br />2. Public Education and Outreach Report <br />A. A copy of all education and outreach materials provided to Customers, or <br />otherwise used for education and outreach efforts in accordance with <br />Section 5.3 of the Agreement, including, but not limited to: flyers, <br />brochures, newsletters, invoice messaging/billing inserts, and website <br />August 17, 2021 133 City of Santa Ana <br />
