ARTICLE XII <br />12.0 EMPLOYEE INSURANCE <br />12.1 Health Insurance. The City shall contribute an allowance towards the employee's Cafeteria <br />Benefit plan for health insurance premiums for eligible employees and their qualified <br />dependents under The Public Employees' Medical & Hospital Care Act (PEMHCA) which <br />governs the California. Public Employees' Retirement System (CalPERS) Health Insurance <br />Programs pursuant to the provisions below: <br />A. For each such employee who is covered under a spouse's non -City sponsored health <br />plan, the City shall pay the employee a cash payment each month in an amount <br />equal to fifty percent (50%) of the monthly premium amount for the City's lowest <br />"employee -only" coverage, if the employee waives, in writing, City -paid coverage. <br />If an employee waives City provided coverage, said employee shall provide proof <br />of medical insurance coverage in a non -City -sponsored health plan. <br />Effective the first full pay period following City Council approval of this 2022- <br />2025 MOU, the City shall pay the employee a cash payment (subject to taxation as <br />wages) each month in an amount equal to the City contribution for the "employee - <br />only" coverage for medical, if the employee waives, in writing, City -provided <br />coverage. <br />Effective January 1, 2022, the City will provide a monthly contribution to <br />health insurance premiums at the following rates: <br />Employee Only $804.00 <br />Employee+l $1,457.00 <br />Employee + Family $1,850.00 <br />2. Effective the first full pay period following City Council approval of this <br />2022-2025 MOU, the City will provide a monthly contribution to health <br />insurance premiums at the following rates: <br />Employee Only $904.00 <br />Employee+l $1,557.00 <br />Employee + Family $1,950.00 <br />3.
