d.  Disparity Impact Statement (DIS).  No‍later‍than‍30‍days‍after‍the‍project‍period‍start‍date,‍you‍ <br />must‍submit‍a‍Disparity‍Impact‍Statement‍(DIS)‍prepared‍according‍to‍the‍guidance‍posted‍by‍the‍ <br />Office‍of‍Minority‍Health‍at https://minorityhealth.hhs.gov/disparities-impact‍‍.‍‍  <br />e.  Audit Findings. ‍No‍later‍than‍30‍days‍after‍the‍project‍period‍start‍date,‍you‍must‍submit‍your‍ <br />corrective‍action‍plan‍for‍any‍material‍weakness‍or‍material‍non-compliance‍finding‍that‍relates‍to‍ <br />internal‍controls‍or‍subrecipient‍monitoring‍in‍your‍most‍recent‍Single‍Audit.  <br />4.First 60 Days. ‍The‍follwing‍item‍must‍be‍completed‍and‍submitted‍as‍a‍Grant‍Note(s)‍in‍Grant‍ <br />Solutions‍within‍the‍first‍60‍days‍of‍the‍project‍period.  <br />a.  Planning Documents. ‍No‍later‍than‍60‍days‍after‍the‍project‍period‍start‍date,‍you‍must‍submit‍a‍ <br />detailed‍work‍plan,‍a‍health‍literacy‍plan‍incorporating‍the‍National‍standards‍for‍Culturally‍and‍ <br />Linguistically‍Appropriate‍Services‍in‍Health‍and‍Health‍Care,‍and‍an‍outcome‍evaluation‍plan. ‍The‍ <br />plans‍should‍appropriately‍relate‍to‍and‍build‍upon‍each‍other.  <br />b.  Subrecipient Monitoring Plan. ‍No‍later‍than‍60‍days‍after‍the‍project‍period‍start‍date,‍you‍must <br />submit‍a‍comprehensive‍subrecipient‍monitoring‍plan‍specific‍to‍the‍subrecipient‍activities‍under‍this‍ <br />project. <br />5.First 90 days.  The‍follwing‍item‍must‍be‍completed‍and‍submitted‍as‍a‍Grant‍Note‍in‍Grant‍ <br />Solutions‍within‍the‍first‍90‍days‍of‍the‍project‍period.  <br />a.  Final Confidentiality Plan. ‍No‍later‍than‍90‍days‍after‍the‍project‍period‍start‍date,‍you‍must‍ <br />submit‍a‍final‍confidentiality‍plan.  <br />6.Documentation of Agreements.‍You‍are‍expected‍to‍submit‍all‍signed‍Memoranda‍of‍Understanding <br />(MOUs)‍with‍all‍partners‍within‍the‍first‍60‍calendar‍days‍of‍the‍project‍period.‍ Additional‍partners‍ <br />can‍and‍should‍be‍added‍throughout‍the‍duration‍of‍the‍award,‍at‍which‍point‍signed‍MOUs‍should‍be <br />submitted‍within‍10‍calendar‍days‍of‍newly‍established‍formal‍partnerships.‍MOUs‍should‍detail‍ <br />roles‍and‍responsibilities‍of‍each‍partner. <br />You‍must‍submit‍a‍notification‍for‍each‍executed‍contract‍that‍includes‍a‍brief‍description‍of‍the‍ <br />contract‍scope‍of‍work,‍cost‍breakdown,‍the‍date‍executed,‍and‍the‍performance‍period‍of‍the‍ <br />contract. ‍The‍notification‍is‍due‍10‍calendar‍days‍following‍execution‍of‍the‍contract. ‍You‍should‍ <br />not‍send‍a‍copy‍of‍the‍contract.  <br />MOUs,‍contract‍notifications,‍and‍any‍other‍supporting‍documentation‍for‍partners‍and‍subrecipients‍ <br />must‍be‍submitted‍via‍Grant‍Notes‍Module‍in‍GrantSolutions. <br />7.Institutional Review Board (IRB). Institutional‍Review‍Board‍(IRB)‍approvals,‍when‍applicable,‍ <br />must‍be‍submitted‍via‍Grant‍Solutions‍Grant‍Notes‍within‍5‍business‍days‍of‍receipt‍from‍the‍IRB. ‍ <br />No‍activities‍that‍require‍IRB‍approval‍may‍take‍place‍prior‍to‍your‍receipt‍of‍the‍IRB‍approval.  <br />STANDARD TERMS <br />1.Compliance with Terms and Conditions. You‍must‍comply‍with‍all‍terms‍and‍conditions‍outlined‍ <br />in‍the‍grant‍award,‍including‍grant‍policy‍terms‍and‍conditions‍contained‍in‍applicable‍Department‍of <br />Health‍and‍Human‍Services‍(HHS)‍Grant‍Policy‍Statements‍(GPS),‍(note‍any‍references‍in‍the‍GPS‍ <br />to‍45‍C.F.R.‍Part‍74‍or‍92‍are‍now‍replaced‍by‍45‍C.F.R.‍Part‍75,‍and‍the‍SF-269‍is‍now‍the‍SF-425),‍ <br />and‍requirements‍imposed‍by‍program‍statutes‍and‍regulations,‍Executive‍Orders,‍and‍HHS‍grant‍ <br />4Page <br />Notice of Award <br />Award# <br />FAIN# <br />Federal Award Date:06/18/2021 <br />1 CPIMP211286-01-00 <br />CPIMP211286 <br />DEPARTMENT OF HEALTH AND HUMAN SERVICES <br />Office of the Secretary
