Adding the following notation to the list of footnotes at the <br />end of Section 3: <br /> <br />(g) Effective January 1, 1980, personnel designated <br />as Executive (~4) and Middle-Management (MM) who meet <br />or exceed special performance criteria established by <br />the City Manager may be granted, upon recommendation <br />of the City Manager and approval of the City Council, <br />a performance incentive allowance in an amount equal <br />to one percent (1.0%) (2 salary rate ranges) above <br />their then current base monthly salary rate. <br /> <br /> 2. That the following subsection be added to Section 3.1.1, Employee <br />Benefits: (for Executive Management personnel): <br /> <br /> k. Life Insurance Coverage. From and after September 1, 1979, or <br />as soon thereafter as practicable, the City will seek to ~end the group term <br />life insurance policy it maintains on behalf of its employees in order to pro- <br />vide each person designated as Executive Management (EM) with term life insur- <br />ance coverage in an amount equal to twice the employee's annual rate of salary, <br />provided said person can provide evidence of insurability if so required. <br />Said life insurance coverage shall include an Accidental Death and Dismember- <br />ment provision. The City will pay any reasonable additional premium cost to <br />provide such life insurance coverage at no cost to the employee. <br /> <br /> In the event an Executive Management employee is determined to <br />be ineligible for said ins.luaace coverage, the City will attempt to provide <br />as much additional coverage ds ,~y be obtained without having to provide <br />evidence of inJ~rability, but in no case shall the amount of coverage be <br />less than the existing $20,000 A.D. and D. coverage. <br /> <br /> 3.
