RESOLUTION NO. 80-89 <br />PAGE THREE <br /> <br />the residence or any part thereof, or <br />any interest thereon, is sold, agreed <br />to be sold, conveyed or alienated by <br />said officer, or by operation of law <br />or otherwise, the entire amount of <br />principal and interest shall become <br />immediately due and payable; and in <br />the event said officer resigns or is <br />dismissed from office, or for any <br />other reason vacates such office, the <br />entire amount of principal and interest <br />shall become due and payable within <br />ninety (90) days of resignation, dis- <br />missal or vacation. <br /> <br />iii. The city shall provide as addi- <br />tional compensation in the event tem- <br />porary housing is required by said <br />officer, all expenses for such tem- <br />porary housing which exceed the sum <br />of One Thousand Dollars ($1,000.00) <br />per month, provided, however, in no <br />event shall such sum exceed Five Hundred <br />Dollars ($500.00) per month. <br /> <br />(f) Movin~ Expense <br /> <br /> The city shall pay for all reasonable <br />and necessary moving expenses incurred by <br />said officer in relocating from the State <br />of Maine to the City of Santa Ana. <br /> <br />(g) Dental Insurance <br /> <br /> The city will contribute up to a max- <br />imum of Fifteen Dollars Thirty-two Cents <br />($15.32) per month toward the payment of <br />premiums for dental insurance plans pro- <br />vided by the city for said officer and any <br />eligible dependents. <br /> <br />(h) Life Insurance <br /> <br /> The city will seek to amend the group <br />term life insurance policy it maintains on <br />behalf of its employees in order to provide <br />said officer with term life insurance cover- <br />age in an amount equal to twice said officer's <br />annual rate of salary, provided said officer <br />can provide evidence of insurability if so <br />required. Said life insurance coverage <br />shall include an Accidental Death and Dis- <br />memberment (AD & D) provision. The city <br />will pay any reasonable additional premium <br />cost to provide such life insurance coverage <br />at no cost to said officer. <br /> <br /> In the event said officer is deter- <br />mined to be ineligible for life insurance <br />coverage in an amount equal to twice the <br />annual rate of salary, the city will attempt <br />to provide as much additional coverage above <br />existing amounts as may be obtained without <br />said officer having to provide evidence of <br />insurability, but in no case shall the <br />amount of coverage be less than the existing <br />$20,000.00 AD & D coverage. <br /> <br /> <br />
