City and Contractor entered into an Agreement for the Provision of Inmate Medical Services, dated September 1, 1998, hereinafter referred to as "said Agreement". B. The Parties entered into an Amendment, dated August 20, 2001, to extend the term of said Agreement and increase the maximum obligation to be expended for services. C. City and Contractor wish to again extend the term and increase the maximum obligation to be expended for services provided pursuant to said Agreement. NOW, THEREFORE, in consideration of the mutual and respective promises, and subject to all the terms and conditions of said Agreement, and Amendments to Agreement, except as herein modified, the parties agree as follows: 1. The Referenced Contract Provisions shall be amended to read as follows: a.) The term shall be extended to August 31, 2003. b.) The Maximum Baseline Obligation shall be increased to $722,040.00 as set forth in Exhibit A, attached hereto and made a part hereof by reference. The amount the City is obligated to pay for services which are included in Exhibit D - Cost Schedule for Chargeback Services shall not exceed ($105,500.00). The total obligation of the City shall not exceed Eight Hundred Twenty Seven Thousand, Five Hundred Forty Dollars ($827,540.00), annually, during the term of this Agreement. 2. Except as hereinabove modified, all terms and conditions of said Agreement shall remain in full force and effect.
