' <br /> <br /> AND GLE~d< OF <br /> CORRECTIONAL MANAGED CARE MEDICAL CORPORATIONDsi E: 9"/7 "al <br /> <br />1 <br />THIS SECOND AMENDMENT TO AGREEMENT, is entered into this ~a-tA day of <br />August, 2001, by and between the City of Santa Ana, a charter city and municipal corporation <br />organized and existing under the Constitution and laws of the State of California ("City"), and <br />Correctional Managed Care Medical Corporation ("Contractor") <br /> <br />RECITALS: <br /> <br />A. City and Contractor entered into an Agreement for the Provision of Inmate Medical <br /> Services, dated September 1, 1998, hereinafter referred to as "said Agreement". <br /> <br />The parties amended said Agreement by a writing dated November 20, 2000, to establish <br />a program for the provision of off-site emergency medical care for Santa Aha Jail inmates <br />and on-site emergency psychiatric crisis intervention and evaluation. <br /> <br />C. City and Contractor wish to extend the term of said Agreement for one year. <br /> <br />D. City and Contractor wish to increase the maximum obligation to be expended for services <br /> provided pursuant to said Agreement. <br /> <br />NOW, THEREFORE, in consideration of the mutual and respective promises, and subject to all <br />the terms and conditions of said Agreement and Amendment to Agreement, except as herein <br />modified, the parties agree as follows: <br /> <br />1. The Referenced Contract Provisions shall be amended to read as follows: <br /> <br />a.) The term shall be extended to August 31, 2002 <br /> <br />b.) The Maximum Baseline Obligation shall be increased to ($656,400.00) as set forth in <br />Exhibit A, attached hereto and made a part hereof by reference. The amount the City is <br />obligated to pay for services which are included in Exhibit D - Cost Schedule for <br />Chargeback Services shall not exceed ($105,500.00). The total obligation of the City <br />shall not exceed Seven Hundred, Sixty One Thousand, Nine Hundred Dollars <br />($761,900.00), annually, during the term of this Agreement. <br /> <br /> <br />
