Santa Ana Blvd. <br />Santa Ana, CA 92701 <br /> <br />CONTRACTOR and ADMINISTRATOR may agree in writing to add, change, <br /> <br />6.2 <br /> <br />modify, or delete facility location(s) as necessary to best serve the needs <br />of participants and ADMINISTRATOR. <br /> <br />7. <br /> <br />CASE RECORDS <br /> <br />7.1 <br /> <br />CONTRACTOR shall maintain a current and complete electronic case <br /> <br />record for each participant referred. <br /> <br />CONTRACTOR shall also maintain a hard <br /> <br />copy case record as well. <br /> <br />The contents of the hard copy case records will be <br /> <br />in a format approved by ADMINISTRATOR and will be uniform among subcontractors <br />and other educational providers. <br /> <br />7.2 <br /> <br />Such i nformati on sha 11 be treated as confi denti a 1 and released <br /> <br />only to ADMINISTRATOR as required or to others upon the written approval of <br />ADMINISTRATOR. <br /> <br />7.3 <br /> <br />CONTRACTOR shall obtain and maintain in the case record a written <br /> <br />release, on an ADMINISTRATOR approved form. from participant authorizing the <br />exchange of confidential information among CONTRACTOR, MDT, and subcontractors <br /> <br />or other educational <br /> <br />providers. <br /> <br />All <br /> <br />subcontractors <br /> <br />shall <br /> <br />treat all <br /> <br />confidential information in the same manner as CONTRACTOR, and in adherence to <br />the terms of this Agreement. <br /> <br />8. <br /> <br />REPORTS <br />8.1 CONTRACTOR <br /> <br />shall prepare and submit to ADMINISTRATOR's designated <br /> <br />staff written reports. including, but not limited to: <br /> <br />8.1.1 <br /> <br />Number of new cases referred each month: <br /> <br />(WSH0204) <br /> <br />5 of 10 <br />
