CONTINUUM OF CARE NARRATIVE <br />HUD has placed significant emphasis in meeting the needs of the homeless through a <br />Continuum of Care system. A Continuum of Care is a comprehensive and coordinated <br />housing and service delivery strategy that responds to the different needs of homeless <br />and at -risk individuals and families, and helps them attain self - sufficiency. As a <br />recipient of CDBG, HOME, ESG and HOPWA funds, Santa Ana supported numerous <br />public and nonprofit programs that reach out to the homeless, brings them into the <br />system of care, and facilitate their movement toward self - sufficiency. Actions <br />undertaken during the 2003 -2004 report period are outlined below. Detailed summaries <br />of HUD - funded activities are found in IDIS reports incorporated into this CAPER. <br />1. ACTIONS TAKEN TO ADDRESS THE NEEDS OF HOMELESS PERSONS AND THE SPECIAL NEEDS <br />OF PERSONS THAT ARE NOT HOMELESS BUT REQUIRE SUPPORTIVE HOUSING (INCLUDING <br />PERSONS WITH HIV /AIDS: Santa Ana provided substantial CDBG, ESG and HOPWA <br />resources to nonprofit agencies for homelessness prevention, emergency shelter, <br />transitional housing, and supportive services. The table below provides an overview <br />of how ESG funds were utilized during the 2003 -2004 report period. <br />City for ESG program admin. <br />A total of 65 persons with HIV /AIDS were assisted with tenant based rental <br />assistance funded with HOPWA resources. Additionally, HOPWA funds were <br />utilized to assist 78 individuals with transitional housing, 551 households with <br />emergency rental /utility assistance, 798 individuals with hospice care, and 52 <br />individuals with detox drug treatment services. <br />2.
