■ Actions taken to prevent homelessness - Santa Ana provided nonprofit homeless <br />service providers with $17,000 in ESG funds during the report period in order to <br />prevent 426 individuals from becoming homelessness. Agencies providing these <br />services included the City's Parks, Recreation and Community Services Agency <br />and the Southwest Community Center. No CDBG funds were allocated for <br />homelessness prevention programs during the report period. On a countywide <br />basis, HOPWA funds were utilized to assist 551 persons with HIV /AIDS from <br />becoming homelessness. <br />■ Actions taken to address emergenc sy helter - ESG funds were utilized to provide <br />emergency shelter and services for the City's homeless. 8 A total of 1,225 <br />individuals were assisted during the report period with ESG funds. Grant funds <br />were used to offset shelter operating costs and to provide essential services for <br />the homeless. CDBG funding was also used to support emergency shelter <br />services. Shelter for the Homeless utilized $5,000 to assist 22 persons, and <br />Orange Coast Interfaith Shelter assisted 382 persons with $5,000.9 <br />■ Actions taken to address transitional housing: $50,500 in CDBG funding was <br />used to provide transitional housing opportunities for 114 Santa Ana homeless <br />individuals. ESG funds provided transitional housing for 835 individuals.10 <br />Regionally, 78 individuals were assisted with HOPWA- funded transitional <br />housing and support services during the 2003 -2004 report period. <br />■ Actions taken to transition homeless into permanent housing: Providers of <br />transitional housing are the key to preparing the homeless for self - sufficiency <br />and movement into permanent housing. During the report period no CDBG or <br />HOME funds were utilized to specifically provide permanent housing; however, <br />HOPWA resources were utilized to assist 133 individuals access and maintain <br />permanent housing with tenant based rental assistance. <br />■ Actions taken to address the needs of sub populations L.g. those living on the <br />streets, and new funds such as Continuum of Care Super NOFA • Several <br />homeless programs supported by the City of Santa Ana have an outreach <br />component for special needs homeless sub - populations. These programs <br />provide access to resources by referral and /or direct contact. These programs <br />include: <br />■ Dayle McIntosh Center — assisted homeless and at -risk individuals with <br />physical disabilities to maintain or obtain housing. <br />8 ESG in the amount of $75,900 was provided for essential services and $179,750 was provided for facility <br />operations. Funds were utilized for both emergency shelter and transitional housing programs. <br />9 Orange Coast Interfaith served 382 persons with ESG and CDBG. <br />10 The referenced CDBG and ESG accomplishments do not take into account an overlap of 114 persons <br />served with both funding sources. Mercy House, Wise Place and Thomas House received both CDBG <br />and ESG funding (totals of $50,000, $80,500 and $24,000 respectively); they assisted 71, 11 and 32 <br />persons respectively. <br />15 <br />19E -18 <br />9/9/04 <br />
