■ Women's Transitional Living Center, Interval House and Human Options - <br />focused program efforts on victims of domestic violence. <br />■ WISEPlace — provided transitional shelter opportunities for homeless elderly <br />women. <br />■ Mental Health Association of Orange County - worked with the City's mentally <br />ill and substance addicted homeless population. Services provided to these <br />chronic homeless populations included supportive services and access to <br />appropriate housing. <br />■ Orange County Health Care Agency - operated a proactive outreach program <br />to assist homeless persons with HIV /AIDS living on the streets. A portion of <br />this program was funded with HOPWA. <br />■ HOPWA Tenant Based Rental Assistance — Funding to assist individuals <br />living with HIV /AIDS maintain independent living situations. <br />Santa Ana did not apply for Continuum of Care Super NOFA funds in 2003 or 2004; <br />however, the City did support the NOFA application of one local nonprofit by <br />providing a Certification of Consistency with the City's Consolidated Plan. (A <br />certification was provided to the Consumer Credit Counseling Services of Orange <br />County for their application to HUD for Housing Counseling grant funds.) The City <br />of Santa Ana also participated in the development of the regional Continuum of <br />Care strategy by providing valuable mainstream fund information and assigning key <br />staff to participate in the Orange County Continuum of Care Community Forum. <br />The Community Forum was responsible for establishing the gaps and priority needs <br />for the County's 2003 and 2004 Associated application for Super NOFA homeless <br />funds. At the local level, a Motel Study was completed, which evaluated the impacts <br />of a recently approved ordinance that may place several "hotel" families at risk of <br />becoming homeless. <br />E. OTHER ACTIONS <br />1. ACTIONS TO ADDRESS OBSTACLES TO MEETING UNDERSERVED NEEDS: According to the <br />City's 2000 -2004 Consolidated Plan, Santa Ana's under - served include the elderly <br />and frail elderly, persons with HIV /AIDS, persons with physical or mental disabilities, <br />youth, homeless and extremely low- income persons. During the report period, the <br />City continued its efforts to address the needs of these populations. These efforts <br />include the following: <br />■ The City continued support for housing for the elderly and frail elderly. There are <br />855 senior only units in various senior housing projects throughout the City. The <br />Council on Aging of Orange County's Linkages and Ombudsman programs <br />ensured 1,654 seniors living in rest/guest homes were provided safe and decent <br />16 9/9/04 <br />19E -19 <br />
